What Information Should I put for Primary Insurance Holder

If you are the primary insurance holder, enter your insurance ID, name, date of birth, preferred name (if any), and gender.

If you are on another individual's insurance plan (such as your spouse), enter their insurance ID, name and DOB into the primary insurance holder field. Then enter your name, preferred name (if any), DOB, and gender in the 'Your information' section. The information you enter in Your Information is what will be used to create your account.
